Book an Initial Session

A 90-minute initial session with me with patients who use Health Insurance (Aetna, Cigna, Blue Cross Blue Shield, or United Healthcare) for Insurance. I recommend calling your insurance Customer Service number to verify benefits prior to our appointment. (see instructions below.)

Book a Follow-Up Session

(Using Health Insurance)

Follow-Up Nutrition counseling sessions of 60-minutes or 30-minutes. This link is for patients using Health Insurance (Aetna, Cigna, Blue Cross Blue Shield, or United Healthcare) and patients that have already been seen by me previously.

Book a Cash-Pay Visit

This is the option to select if you do not want to work via insurance. Book a 90-minute initial session with me. Cost for initial visits are $170.


Book a Cash-Pay follow-up session

This is the option to select if you do not want to work via insurance60-minute follow-up Nutrition Counseling/Medical Nutrition Therapy. Cost is $140/hour


Instructions for Checking Insurance Benefits

In an effort to streamline the process of billing insurance for our services, we are asking you to call your own insurance
carrier to inquire about the details of your benefits.

I am in network with Aetna, Cigna, Blue Cross Blue Shield and United Healthcare.

For any other insurer, you might ask about Out-of-Network benefits.

You will want to have the following info handy when you make the call:

  • Client Name (person to be seen by On Kate’s Plate LLC)

  • Date of Birth

  • Member ID (from your insurance card)

  • Provider Name: On Kate’s Plate LLC or Katherine Dwyer

  • Provider Tax ID: 93-4467501

  • Provider NPI: 1508630591 (For BCBS you will need to provide the Type 2 NPI 1952118879)

  • CPT/Procedure Codes: 97802, 97803

  • Diagnosis Codes: Start by asking about Z71.3. Other codes (from your provider, for example hypertension, IBS, obesity) If you need help determining these ask your primary care provider.

1. Is this provider (me!) in-network with my plan?

Ask about this by providing my NPI number. For any UHC, Aetna, or Cigna plans you will provide the number 1508630591.

For any BCBS Plans, please use the NPI 1952118879.

They may also ask for my Tax ID number which is 93-4467501

2. Do I have nutritional counseling benefits under the following CPT and diagnosis codes?

When calling, ask if your plan covers Medical Nutrition Therapy (MNT) using these standard billing codes:

Initial session: CPT code 97802

Follow-up sessions: CPT code 97803, or 99404

Diagnosis code (ICD-10): Z71.3

Be sure to ask about preventive care vs. medical care coverage.

Preventive care is often covered at 100%, while medical care may require a copay, deductible, or coinsurance.

You can also check coverage under a BMI diagnosis code. To find the right one:

1. Calculate your BMI using an online BMI calculator (you’ll need your current height and weight).

2. Google “ICD-10 code for BMI XX kg/m2” using your BMI number to get the appropriate code.

3. Do I have any nutritional counseling benefits covered under the preventative care portion of my plan?

Ask “Are nutrition visits covered for preventative care with any specific conditions?”

4. What is my responsibility (deductible, copay, coinsurance)?

If deductible applies, how much has been met?

5. Is there a visit limit?

You will want to ask about how may visits are covered under your plan. Some plans cover 3 visits, some have unlimited visits.

6. Is telehealth covered? If so, is my responsibility the same:

Is there an expiration date for telehealth?

I currently ONLY see patients via telehealth, so if this is not covered unfortunately we will not be able to work together.

7. Do I need a referral?

8. Is a prior-authorization required? If so, what is the process?

9. Can I have your name and a reference number?

Document the date/time of the call for future reference!
